What Is Protected Health Information (PHI)

Protected health information (PHI) is individually identifiable health information, including details about your past, present, or future physical or mental health, the care you receive, and payment for that care. PHI may be in written, electronic, or spoken form.

How We May Use & Disclose PHI

  • Treatment: We may use and share your PHI to provide, coordinate, and manage your care, including with providers, labs, and pharmacies involved in your treatment.
  • Payment: We may use and share your PHI to obtain payment for the services we provide, such as billing and processing payments.
  • Healthcare Operations: We may use and share your PHI for operations such as quality improvement, scheduling, staff training, and general administration of the practice.

Other uses and disclosures may be required or permitted by law, or made with your written authorization. You may revoke an authorization in writing at any time.

Electronic & Text Communications

We may use phone, email, and SMS text messages to communicate with you about appointment reminders and to coordinate your care. Standard SMS and email are not encrypted, so we limit the protected health information we send electronically. You may request that we communicate with you by alternative methods or at an alternative location, and we will accommodate reasonable requests. Your Health Information Rights

  • Access: You may inspect and request a copy of your health and billing records, with limited exceptions.
  • Amend: You may request that we amend health information you believe is incorrect or incomplete.
  • Accounting of disclosures: You may request a list of certain disclosures we have made of your PHI.
  • Request restrictions: You may request limits on how we use or disclose your PHI; we are not required to agree to all requests.
  • Confidential communications: You may request to receive communications by a specific method or at a specific location.
  • Paper copy: You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.
  • Complaints: You may file a complaint if you believe your privacy rights have been violated (see below).

Our Responsibilities

We are required to maintain the privacy and security of your PHI, to notify you following a breach of unsecured PHI as required by law, to follow the duties and privacy practices described in this notice, and to provide you with a copy of this notice. We will not use or share your information other than as described here unless you tell us we can in writing.

Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as information we receive in the future. The current notice will be available at our office and, in summary form, on this website, with the effective date shown above.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our practice using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
