We are required to maintain the privacy and security of your PHI, to notify you following a breach of unsecured PHI as required by law, to follow the duties and privacy practices described in this notice, and to provide you with a copy of this notice. We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as information we receive in the future. The current notice will be available at our office and, in summary form, on this website, with the effective date shown above.
